Explanation:

Aldolase, AST, ALT. This patient most likely has juvenile dermatomyositis (JDM), which typically affects young boys between the ages of 5-10 yo presenting with proximal muscular weakness. The muscle weakness can involve the diaphragm and esophageal musculature, causing symptoms of acid reflux. A diffuse photosensitive rash is present in 25% of patients, and in contrast to the malar rash of SLE, the rash of JDM involves the nasolabial folds. An additional classic skin finding in JDM is Gottron papules (as shown in the photo), which are dark red lesions found on the extensor surfaces of the hands, elbows and knees. Laboratory studies that are abnormal in JDM are the muscle-derived varieties: Aldolase, AST, ALT, LDH and CPK. Abnormalities of C3 and C4 are more common with SLE.

Anti-Ro antibodies, anti-smooth muscle antibodies, and anti-double stranded DNA antibodies can be abnormal in various autoimmune conditions, such as systemic lupus erythematosus, Sjögren’s syndrome, and autoimmune hepatitis.
