High-Yield NP Questions

AANP-style, blueprint-based Q&A.
Just like you’ll see on the actual AANP exam.

Positively-stated, Case-based Question Stems

No “negatively phrased” questions, no “A and B”…you know what we mean. Our questions always address important “key point” blueprint-based content in a well structured manner.

A 45-year-old man presented to you 3 months ago with leg weakness and ataxia. You performed a physical examination, which revealed clonus and hyperreflexia. Magnetic resonance imaging (MRI) of his spine is shown in Figure 1; MRI of the brain performed at the same time was normal.

Figure 1

Today, the patient is presenting to you with complaints of fatigue, blurry vision, and incontinence. Repeat MRI of the brain is shown below (see Figure 2)

Figure 2

A diagnosis of multiple sclerosis (MS) can be made based on fulfilling which of the following criteria?

Clear Answer Options

A question’s difficulty is defined by the choice of distractors. Good distractors determine the difficulty level of a question. Therefore, good distractors are one of the most important features of a high quality question.

radiographic evidence of a single lesion occurring ≥ 2 times
radiographic evidence of ≥ 2 lesions occurring simultaneously in time
radiographic evidence of ≥ 2 lesions occurring ≥ 2 times
radiographic evidence of multiple brain lesions regardless of time of occurrence
radiographic evidence of spinal cord lesion with clinically consistent symptoms regardless of time of occurrence

High-yield Remediation

Understanding why an answer choice is incorrect is just as important as knowing why it is correct. Med-Challenger questions contain detailed explanations for the correct and incorrect answer choices along with integrated media for those that learn best by visual stimuli.

Explanation:

No definitive diagnostic laboratory test exists for MS, but MRI of the brain can be extremely useful and should be performed in all patients when MS is in the differential diagnosis.

More than 95% of patients with clinically definite MS have an abnormal result on a brain MRI. Specific findings on MRI can provide evidence of disease disseminated in time and space (different parts of the brain or spine). MS plaques typically appear as high-signal (white) areas on T2-weighted images. Features of lesion seen on MRI suggestive of MS include an elliptical shape, discrete borders, lack of mass effect, and gadolinium enhancement. Typical locations include the periventricular area (as seen on the brain MRI), the corpus callosum, the cerebellar peduncles, the brain stem, the juxtacortical area, and the dorsolateral spinal cord (as noted on the MRI of his spine).
