Here's this week's sample CME board review question on hematemesis.

A 12-year-old, otherwise healthy boy presents for evaluation of hematemesis.

His parents state that he has no medical or surgical history, no allergies, and has had no recent travel, history of trauma, or sick contacts.

He awoke this morning complaining of nausea and vague abdominal pain, and could not eat breakfast. At lunchtime, he had two episodes of coffee-ground emesis. 

Upon physical examination, he is awake, alert, and cooperative, with a heart rate of 90 beats per minute and blood pressure of 110/65 mmHg.

His abdomen is soft, with minimal tenderness to palpation in the epigastrium without rebound or guarding.

Stool examination shows melena that is guaiac positive.
