Here's this week's sample CME board review question on immunodeficiency disorders.
A 32-year-old patient presents to you with nasal congestion, purulent nasal discharge, and upper teeth tenderness for 1 week.
On examination, he is febrile, has a nasal voice, and tenderness to palpation of upper teeth. He has chronically decreased audition from the left ear.
He explains that he has "always been unlucky with infections" and describes numerous episodes compatible with sinusitis and otitis ever since he was a child.
In the last year he had 2 episodes of pneumonia, including 1 caused by Pneumococcus infection.
A fourth-generation HIV test is negative.
His immunoglobulin (Ig) G level is 210 mg/dL.
