Here's this week's sample CME board review question on environmental disorders.

A 23-year-old woman presents to you following a scuba class held in a local lake.

She and other students were practicing "buddy breathing" and mask clearing in 16 to 20 feet of water, which is when she suddenly became agitated, dropped her tank, and shot to the surface, where she was pulled out of the water unconscious.

The patient regained consciousness after approximately 2 minutes and stated that she does not know what happened.

Upon presentation, she complains of feeling dizzy with vague chest discomfort.

Her examination is normal, as are findings on electrocardiography and chest x-ray.

Complete blood count is normal.

A pregnancy test is negative.


Which of the following best describes the management of this patient's condition?

Answer Options:

She was not deep enough for decompression sickness (DCS) or other diving-related problems. She should be admitted for syncope of unknown etiology and possible seizure disorder.

She should be treated as a near-drowning case and observed for 6 hours with chest x-ray repeated after 6 hours.

She should be immediately transferred to the nearest hyperbaric center for recompression therapy.

Magnetic resonance imaging (MRI) or computed tomography (CT) should be performed to look for intracranial pathology that could explain her symptoms.
