Here's this week's sample CME board review question on Covid in Pregnancy...
A 26-year-old pregnant female presents at 35 weeks gestational age with a history of COVID-19 symptoms for 10 days and a positive SARS-CoV-2 PCR test 2 days ago.
She now presents with ongoing high fevers, worsening myalgias, and significant shortness of breath.
A workup reveals pneumonia with hypoxia (89% pulse oximeter saturation on room air).
She improves to 92% with high-flow nasal cannula (HFNC) oxygenation.
A team member wonders about awake prone positioning for this patient.
Question:
What do the National Institute of Health (NIH) COVID-19 treatment guidelines recommend with regards to awake prone positioning versus intubation of pregnant patients in their third trimester with COVID-19 and ongoing hypoxemia as described in the above patient?
Answer Options:
In pregnant patients with ongoing oxygen saturation <93% despite HFNC, intubation is recommended over awake prone positioning.
A trial of awake prone positioning is recommended only during the first trimester.
A trial of awake prone positioning is recommended up to the end of the second trimester. During the third trimester, intubation rather than prone positioning is the correct next step.
A trial of awake prone positioning is acceptable during all 3 trimesters and, depending on the trimester, can be performed either in the left lateral decubitus position or the fully prone position.
