A 44-year-old woman presents to you with increasing confusion and lethargy. Her symptoms began approximately 24 hours prior to her presentation according to the patient’s husband. The patient is unable to appropriately answer questions.
Vital signs are: temperature 102 °F, blood pressure 110/68 mm Hg, pulse 96 beats/minute and regular, and respiratory rate 18 breaths/minute. She appears moderately ill. You notice that there are petechiae present on the lower extremities. The rest of the physical examination does not uncover any abnormalities.
Laboratory values are as follows:
- Hemoglobin: 7.6 mg/dL
- Hematocrit: 22%
- Platelet count:46,000/mm3
- Red blood cell indices: Normal
- Serum creatinine: 4.3 mg/dL
- Peripheral blood smear: Frequent schistocytes
