A 35-year-old woman presents complaining of worsening dyspnea on exertion, orthopnea, and paroxysmal nocturnal dyspnea. She has recently noticed blood-tinged sputum. She reports no notable medical history, and her only medication is a multivitamin. She was born in rural Sudan and immigrated to the U.S. in her early 20s. Her family history is unknown. Her temperature is 98.5 F, heart rate 95, blood pressure 108/72, respiratory rate 18, saturation 99% on room air. She has jugular venous distension to the angle of her jaw. She has a loud S1, and S2 is normally split. She has a quiet apical impulse and a parasternal lift. Her heart is regular, with a 3/6 low-pitched early diastolic rumble at the apex. Her chest is clear to auscultation, with isolated crackles at the bases. Her liver is somewhat enlarged. She has no stigmata of chronic liver disease. She has 2+ bilateral edema in her feet and ankles. Complete blood count, basic metabolic panel, and thyroid-stimulating hormone are normal. Chest x-ray, echocardiogram, MRI and esophagogram show the following:

Educational Objective:

Diagnose mitral stenosis.

Key Point:
Mitral stenosis causes volume overload of the structures (hemodynamically) behind the mitral valve, thus causing left atrial enlargement and left ventricular heart failure. If the disease is long standing the right ventricle becomes hypertrophic and right sided heart failure ensues.

Explanation:

Patients with mitral stenosis (MS) develop typical symptoms of left-sided heart failure. As the disease progresses, right ventricular (RV) failure develops. Hemoptysis is common with MS, but not with other causes of left atrial hypertension. Auscultatory findings in MS may be subtle. They include a quiet apical impulse and a parasternal lift present once RV hypertrophy develops. Loud S1 is due to increased transmitral gradient. S2 is physiologically split and usually followed by an opening snap. The opening snap is followed by the classic low-pitched early diastolic rumble, which increases in length as mitral stenosis worsens. This murmur can be inaudible in a patient with low output. The imaging studies show left atrial enlargement, in this case secondary to MS.
