He reports that he felt well until the day prior, when he noted vague and diffuse abdominal pain that then localized to the right lower quadrant. The pain is worse with ambulation. He also reports nausea and subjective fever, but no vomiting. He reports three episodes of diarrhea today, and notes that although he has no past medical history, he has suffered from intermittent diarrhea for the past three months. His family history is notable for inflammatory bowel disease in a first-degree relative.

On examination, he appears nontoxic, but uncomfortable. Temperature 38.2 °C (100.8 °F), heart rate 90/min, blood pressure 125/80 mmHg, respiratory rate 12/min, oxygen saturation 97% on room air. His abdomen is soft, nondistended, and tender to palpation in the right lower quadrant, with focal voluntary guarding. Rovsing’s and psoas signs are positive. Laboratory work is significant for a white blood cell count of 16,000 per microliter.

Key Point:

While young males with a classic presentation of appendicitis often proceed to the operating room without imaging, a history of diarrhea, and a family history of inflammatory bowel disease, should raise suspicion for terminal ileitis.

Explanation:

At first glance, this patient presents with classic signs of appendicitis, with abdominal pain localizing to the right lower quadrant, anorexia, leukocytosis, and a consistent physical examination. However, his reported history of recent intermittent diarrhea points to an alternative diagnosis, particularly in conjunction with a family history of inflammatory bowel disease (IBD). Terminal ileitis can be a presenting sign of IBD, and in such cases, a CT scan with IV contrast should be obtained to evaluate for bowel inflammation. A presumptive diagnosis of appendicitis would not be appropriate in this case. Clostridium difficile colitis is low on the differential, given no history of recent antibiotics or hospitalization.
